All-Star Uca Music Release?! Help?!

A gym owner I did choreography for is attending UCA for the first time. She had some questions however I was no help. As I have never attended the event. She has some questions about the release form. It says that the music has to be licensed by one of three companies. ASCAP, BMI, SESAC are the three companies. What steps do the gym owners need to take to get there music licensed through them. The information is also asking for a notary with a signature. Would this be the specific music producers signature or do we have to take it somewhere to get it notarized?
 
The individual songs will be licensed through one of those 3 organizations.

The gym owner is going to have to do a search on each song in the team's music. The below websites will have the licensing info. Once the form in filled out, they'll have to bring it to a notary, and sign it in front of them. There is usually a small fee to have something notarized. It's tedious, but not difficult. Hope that helps.
