I only saw one foot go out of bounds after rewinding about 50 times

First running pass with all the boys, he literally bounced out on the rebound, his feet were also together so I'm guessing all out.

It was more than just the basket toss fall.

The base on the left fell to the ground, which forced the base on the right to take longer to travel across the mat. He missed the catch in the pyramid so that's one major BF from a toss and a major PYR BF.

No, out of bounds isn't unless both are off the red. He's fine.

The group falling on the floor is a problem though. :(
