Quick question. Pyramid with 3 stunt groups. Is an extended inside hitch legal? Center flyer is at prep. two outside flyers are at extended hitch with the hitched foot in the center flyer's hand. Connection was initiated at prep and then the 2 outside flyers bump to extension? no other connection involved except the foot in center flyer's hand.

No, the connection has to be arm and arm originating at prep level or below.

Nope. Have the flyers grab hands and hitch on the prep's shoulders.

What you listed was a level 3 pyramid.

You have two options - hand to hand connect and have the outside groups hitch to the preps shoulder or keep everyone at prep level and hitch as you listed without hand to hand contact.
