Mine were caused by two issues that were discovered quite by accident. I was getting frequent sinus infections that led to some CT scans and a scope to see what was going on in my sinus cavity. They discovered I had no frontal sinus cavity which wasn't a bad thing it just meant I would not have a singing career which I already knew. The doctor asked if I had migraines and I told him I in fact did. He said what was happening was as my sinuses would stop up pressure would build but not in the frontal cavity (since I don't have one) but rather the back. The problem became that once the pressure had built in the back cavity there was already a tremendous amount of pressure in my head and that would trigger the migraine. They did surgery to fix severly deviated septums as he called it which has all but eliminated the sinus infections but I also know the warning signs to a possible migraine (stiff, aching neck) and have learned to address them immediately which has helped control them tremendously.
