Someone from F5 did it at Return to Atlantis about 4-5 years ago.

She is on f5 now, but it was when she was on one of the best Youth 5 teams ever (twisters Tinys). She handspringed into a double full landing on the front edge of the mat and fell head first onto a concrete floor. She was ok and so lucky but I still wonder why they change other rules like eliminate tumbling skills but still permit elevated stages without sufficient margins - a huge safety issue.

Just a few weeks ago at American superstars in myrtle beach. Guy threw a double off the mat, off the stage and into the crowd, it was scary.

Alright so 3:17 which team is going to walk out on the mat like that at worlds?:p
I laughed out loud watching that part!

On topic though, I am super paranoid about tumbling off the stage. I'm not doing anything crazy, but I tumble to the front in my teams routine and I feel like if I over rotate at all, I'll be diving face first into the crowd at competitions with stages. It puts a lot of added stress on me to be at competitions with stages. It's fun to be on a stage and everything, but I really don't think it's safe.

Wasn't there a team that had a comp in a swimming pool, or by a swimming pool and one of the athletes fell off backwards into the water. I cant recall properly and I've looked for the vid, am I remembering right that it was a European comp??? Anyone???

One of our boys tumbled off the stage at Jamfest Indy last season...luckily two women were by the stage and caught him and pushed him back on...think its on youtube.

OK this is it. And my brain didn't quite remember it right as they are certainly not cheerleaders in our sense and they are not at a comp either. So I suppose it's an epic fail on my brains part, but a good chuckle anyway ;)

at a competition a few years ago, one of the girls from our gym was doing a front walk over roundoff hand spring stepout roundoff tuck up op one panel, well there was a microphone at the very top of the center panel probably like a foot off the mat (microphone was only about a foot high) well this girl tumbled on to it! quickly got back up and smiled like nothing had happened but a staff member ran right up and fixed the microphone!
