OT Advertising Banners

Hi everyone! I was wondering if anyone knows how to create a 728x90 banner for advertising purposes on the top of the page? I have wasted just under 4 days trying to create one and I truly believe the universe does not wish for me to do this... under any circumstances.. lol If anyone can do this for me or point me in the right direction I would certainly appreciate it!
